Agreement to Mediate
This document explains how mediation works, including confidentiality, impartiality, and voluntary participation. You’ll be asked to review and complete this before your first mediation meeting.
A court-approved financial disclosure form used when resolving financial matters following separation or divorce. Your mediator can explain what information is required and how it is used.
Completed before a Mediation Information and Assessment Meeting (MIAM). It helps your mediator understand your circumstances and assess whether mediation is suitable.
Used by solicitors referring clients, or by individuals referring themselves. The form gathers the information needed to get started, after which we’ll review the details and discuss next steps.
